PROJECT PURPOSE
As the demographics of our patient base and the expansion of Medicaid funding in the state of Oklahoma continue to evolve so does our partnership with Legal Aid of Oklahoma. Funding to provide free services to our clients would add more value to the services we provide at CCMH. Legal Aid is a nonprofit here in the Comanche County area that provides a variety of Care Management services such as full representation, consultations with healthcare providers and training for healthcare staff on SDOH.
EVIDENCE
CCMH has been a partner with Legal Aid for many years this would allow us to expand our program services to more patients when needed.
POPULATION DESCRIPTION
Our community is somewhat transient with Fort Sill, it is also surrounded by some very rural blue collar citizens as well as veterans. This population also includes a variety of Hispanic, and Native American citizens as well. This service is critical at times to help these people gain the services they need through the legal system. Legal Aid also provides training to our hospital staff on how to handle certain legal issues.
PERFORMANCE MEASURING
Measurement will be measured by the number of people that use this legal aid service.
