PROJECT PURPOSE
The purpose of this project is to provide an array of health screenings/testing to include: Chem 25 a complete metabilic panel, HIV/STI, COVID-19. Upon identification of a positive test result the individual will be referred to our clinical arm New Hope Wellness Center for medical treatment and care.

The Oklahoma State Department of Health and the most recent State of the State's Health Report issued by the Oklahoma State Board of Health documents that African Americans and other minorities and marginalized individuals residing in Oklahoma and particularly in Oklahoma and Tulsa counties are disproportionately infected and affected by social determinants of health.
POPULATION DESCRIPTION
The Oklahoma State Department of Health and the most recent State of the State's Health Report issued by the Oklahoma State Board of Health documents that African Americans and other minorities and marginalized individuals residing in Oklahoma are disproportionately infected and affected by social determinants of health. The project would greatly benefit the unemployed and those living below the poverty line.
PERFORMANCE MEASURING
Project success will be evaluated on the number of health screenings, number of persons accessing treatment and ultimately recovering/restore to good health or viral suppression. Programmatic data will be collected through a variety methods including electronic medial records, consent forms, pre and post assessment surveys, and outreach forms.
